Data Controller

The controller of personal data is Tomasz Szary operating as the sole proprietorship Tomasz Szary Table Tennis, 43-502 Czechowice-Dziedzice, ul. Lipowska 72, Tax ID (NIP) 652-164-59-55, REGON 388789917, phone number +48 505 720 869, email address t.szary@gmail.com.

Sharing your personal data with the Controller, including health-related data, is voluntary and is done by giving explicit consent on the website.

The basis for our processing of your personal data is Article 6(1)(a) and (1)(b), and Article 9(2)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR).

Purposes of data processing

The personal data you provide in connection with using an account on the website will be processed for the following purposes: to perform the contract concluded between us (standard data and health data); to safeguard your rights as a party to the contract, for example to handle a complaint (standard data only); for marketing activities, such as sending our commercial information (not applicable to persons under 18) — sending such information electronically is only possible if you have consented to it; to issue sales documents (standard data only); to archive documentation (electronic or paper) relating to contract performance, accounting and taxation (standard data only); to establish and pursue claims arising from the contract concluded between us (standard data only); to create reports, analyses and statistics for our internal needs, in particular reporting, planning the development of sales of goods and services, development work in IT systems, and creating statistical models (standard data only).

To whom do we disclose your personal data?

As a rule, the Controller does not disclose your personal data to any third parties. Exceptions are cases in which transferring standard personal data is necessary to achieve the processing purposes through cooperation with entities supporting our business (subcontractors providing marketing, IT, accounting, legal, debt collection, payment, and banking services).

We assure you that, in the contracts we conclude, we require all entities to which we entrust the processing of your data to comply with the standards arising from applicable law. Your personal data is under no circumstances transferred outside the European Economic Area.

How long do we intend to retain your personal data?

The personal data you have shared with us will be permanently deleted after three months from the end of the last processing purpose for which it was used. The exception is personal data used for sending commercial information electronically (provided you have consented), which is permanently deleted within 60 months from the day you stopped using our services.

Where do we obtain personal data from?

The personal data we process comes primarily directly from you. This includes both standard personal data and special categories (relating to your health). We may also obtain your standard personal data from publicly available registers or from third parties, if this is necessary to achieve the processing purposes.

For example, if you pay for our services through a bank, we will obtain information about the account and institution from which the payment was made — we process this data to verify the correctness of the payment, record it, and settle it.

What rights do you have?

At any time, you have the right to: rectify (correct) inaccurate data or supplement it; restrict the processing of data; request access to your personal data (including: information about the data we process, the purposes of processing, the recipients of the data or categories of recipients) and receive a copy of it; transfer the data to another data controller; erase personal data (the so-called right to be forgotten), if any of the circumstances described in Article 17(1)(a)–(f) GDPR apply; withdraw your consent to processing at any time.

You can exercise these rights by submitting a request electronically to the address t.szary@gmail.com or in any other way that allows the applicant to be identified. The scope of each of these rights and the situations in which they can be exercised result from legal provisions. This means that in some cases we may refuse to grant a request — in such cases we will explain the reasons for our decision and provide the legal basis for it.

Right to object

Regardless of the rights listed above, you may at any time object to the processing of personal data for direct marketing purposes. Once such a request is received, we are required to stop processing the data for that purpose.

Right to lodge a complaint with a supervisory authority

We assure you that we adhere to a strict personal data security policy and that your personal data is safe with us. Nevertheless, please remember your right to lodge a complaint with the President of the Personal Data Protection Office, if you believe that the processing of your personal data violates the law.

Changes to the privacy policy

The Controller reserves the right to change the privacy policy if required by applicable law, if the technological conditions of the website change, or if the change introduces a higher standard than the minimum required by law. The Controller will notify you of any such change with appropriate advance notice via electronic correspondence.

This data is collected solely for statistical purposes to optimise the website and concerns, among other things: the type of device, operating system, type of web browser, screen resolution, colour depth, or internet service provider.
